The Residential Program serves individuals in whichever setting is best suited to their needs, living independently, with family members, or in a state-qualified Host Home.
Position: Reverse Host Home Provider
Location: Lakewood
Compensation & Living Arrangement:
- $165.00 per day non-tax non-reportable income.
- A room and board rate will be negotiated separately with the individual's representative payee.
- The individual receives additional scheduled support during the day, allowing the main provider the opportunity for personal time, errands and other commitments. Support may be shared with another live-in provider or by trained support staff.
Reverse Host Home Overview:
We are seeking a compassionate and dependable Reverse Host Home Provider to share a home with an individual receiving services. In this unique residential support role, the provider moves into the individual's home and becomes part of their daily routine by providing supervision, companionship, and assistance with everyday activities. The goal is to support the individual in remaining safely and comfortably in their own home while promoting independence, dignity, and a high quality of life. This opportunity may be especially well suited for a couple, family members who wish to share caregiving responsibilities, or an individual who already lives independently and is looking for a meaningful live-in caregiving role.
About the Individual:
The individual is a kind-hearted man in his 60s with a wonderful sense of humor who enjoys relaxing at home and watching television. He is nonverbal and prefers the comfort of his home environment. He requires 24/7 supervision, as he cannot be left alone, and benefits from a calm, patient, and consistent caregiver who enjoys building meaningful relationships.
Job Functions:
- Reside in the individual's home and provide a safe, supportive, and nurturing living environment while ensuring their health, safety, and well-being through 24/7 supervision
- Prepare meals and ensure the individual receives proper nutrition throughout the day
- Assist with activities of daily living (ADLs), including providing restroom reminders and assistance with toileting as needed
- Supervise showers to ensure safety and compliance with established protocols (the individual can bathe independently)
- Collaborate regularly with the client’s support team
- Communicate clearly and promptly with the agency regarding any changes, concerns, or incidents
- Act as a coach and mentor, providing guidance in daily activities while ensuring the individual is treated as an adult and encouraged to make her own choices
- Assist with budgeting, managing spending, and ensuring receipts are submitted as required
- Provide transportation and support for medical appointments
Qualifications & Requirements:
- At least 21 years of age
- Able to pass a criminal and Adult Protective Services background check.
- One year supporting adults with intellectual and developmental disabilities (I/DD) is preferred.
- Experience supporting individuals who are nonverbal or use alternative forms of communication is preferred.
- Willingness to reside in the individual's home and provide support in a primarily home-based setting, understanding that the individual prefers to remain at home and requires continuous supervision.
- Ability to remain calm, dependable, and attentive in a home environment requiring continuous supervision.
- Comfortable providing personal care, including toileting assistance when needed.
- Ability to follow individualized care plans and agency protocols.
- Strong communication and organizational skills.
- Valid driver's license, reliable transportation, and acceptable driving record (preferred).
- Maintain CPR/First Aid and QMAP (Medication Administration) Certifications
- Possess a valid driver’s license, reliable transportation and valid vehicle insurance
Physical Requirements:
- Able to provide all elements of hands-on care for the client’s individual needs.
- Bend, sit, reach, and lift up to 50 pounds.
Additional Information:
- Host Home Providers receive a negotiated daily rate for care of the individual(s) in their home, as well as additional room and board payments. These rates are based on the individual's level of care required.
- All adults (not individuals receiving services) who reside in the home, whether they are providing services, must also pass a criminal background check.
