Reports to: Residential Director
Position Purpose: The Residential Program Managers provide case management services to those on the DD waivers, who receive services in their own homes, or in a host home environment.
Position Overview:
Residential Program Managers are responsible for overseeing a caseload of adults with developmental and intellectual disabilities who are receiving services in residential settings, funded through the Home and Community Based (HCBS) Medicaid Waivers. Time is spent both in the office and the community, visiting clients and providers in their homes.
Job Duties:
- Maintain positive relationships with the individuals that we serve, members of their Interdisciplinary Team (IDT), coworkers and family members.
- Provide Intensive Case Management services for assigned clients including but not limited to:
- Maintain client master files
- Ensure all documentation is completed monthly in order to stay in compliance with state guidelines, rules and regulations
- Conduct both monthly announced and unannounced visits in a variety of settings
- Attends all service plan, IDT, intake meetings; and if necessary, medical appointments as needed
- Serve as the primary contact for residential related services
- Assist in training and implementing client’s care plans collaboratively with other team members
- Provide initial and on-going trainings with providers
- Complete monthly internal audit of client finances
- Work with in-house nursing team, assuring that medications are appropriately handled, discrepancies are reported, and proper documentation is completed
- Communicate with Social Security and Medicaid to maintain and establish eligibility for services
- Additional duties as assigned
Qualifications
Education
- Undergraduate degree in Human Services, Special Education, Social Work, or related field; or High school diploma with a minimum of 5 years’ experience

- Ability to prioritize workload and complete tasks in a timely and efficient manner
- Ability to set boundaries, resolve conflict and de-escalate issues
- Dependable, able to work under pressure, receptive to change, displays a willingness to learn along with a cooperative approach to problem-solving
- Possession of a valid Driver’s License and Auto Insurance (travel on behalf of the Agency is required) and able to pass a criminal background check
- Excellent oral and written communication skills in order to effectively communicate at all levels
Physical Requirements
- Prolonged periods sitting at a desk and working at the computer
- Attend meetings that may last up 3 hours in person and remotely
- Must be able to lift up to 15 pounds at a time
- Drive to and from meetings in the community
