Position: Day Services Program Manager

Reports to: Executive Director

Position Overview: The Day Services Program Managers provide case management services to those on the CES, SLS, and DD Medicaid waivers, who receive services in their homes, communities and Day Program.

Job Duties:

1. Build and maintain positive relationships with the individuals that we serve, members of their internal and external teams, coworkers and family members.

2. Contribute to the professional reputation of the agency; adhere to all agency policies, procedures and DIDD regulations

3. Provide Case Management services for assigned clients including but not limited to:

• Maintain client master files
• Ensure all documentation is completed monthly to stay in compliance with state guidelines, rules and regulations
• Conduct monthly and quarterly visits with each client on the caseload
• Check billable contact notes and utilization and EVV of services daily
• Attends all individuals’ service plan and IDT meetings, 6-month reviews, 30-day reviews, and intake meetings
• Assist in training and implementing client’s care plans collaboratively with other team members
• Develop, train and implement ISSP/BISSP goals
• Provide initial and on-going training with providers
• Complete and implement follow-up for all Incident Reports relating to Day Services
• Maintain client specific information using Spectrum’s online tools

4. Additional duties as assigned

Qualifications

Education

Undergraduate degree in Human Services, Special Education, Social Work, or related field; or High school diploma with minimum of 2 years’ program management or coordination experience.

• Dependable, able to work under pressure, receptive to change, displays a willingness to learn along with a cooperative approach to problem-solving
• Possession of a valid Driver’s License and Auto Insurance (travel on behalf of the Agency is required) and able to pass a criminal background check
• Excellent oral and written communication skills to effectively communicate at all levels

Physical Requirements

• Prolonged periods sitting at a desk and working at the computer
• Attend meetings that may last up to 3 hours in person and remotely
• Must be able to lift up to 30 pounds at a time
• Drive to and from meetings in the community
